1. HOW TO ENTER:
Complete the entry form for the sweepstakes to enter. All fields are mandatory. Visit http://www.lifeandstylemag.com/win_it and follow on-screen instructions to complete the official entry form, as indicated, with your first name, last name, complete address (no P.O. boxes), daytime phone number, email address and birth date. Click on the “Submit” button and you will be automatically entered into the selected Sweepstakes. Limit one (1) entry per person and/or email address. If there is a dispute about who the email address or entry belongs to, the entry will be deemed to be submitted by the person whose name is stated on the entry form.

Sponsor is not responsible for transmission errors resulting in omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions or limitations of any kind, whether caused by any of the equipment or programming associated with or utilized in the sweepstakes, or by any technical or human error which may occur in the processing of the entries in the sweepstakes, or for inaccurate transmission of or failure to receive entries due to transmission failures or technical failures of any kind, and such entries will be void. Sponsor reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the sweepstakes. Sponsor is not responsible for computer system, hardware, software, network, program or other damage, errors or failures of any kind, whether human or technical in nature, that may arise from or in connection with participation in this sweepstakes, or in the transmission or receipt of entries.
No automatic, programmed, robotic or similar means of entry are permitted, and individuals engaging in such practices will be disqualified. The use of automated or third-party software or Web site
to enter is prohibited. All entries become the sole property of the Sponsor and will not be acknowledged or returned. Incomplete entry forms will be disqualified.
2. ODDS OF WINNING:
Odds of winning will be determined by the total number of eligible entries received.
3. PRIZE(S):

5. House of Hype Sundance Gift Bag. One (1) prize will be awarded. Prize include Oakley Limited Edition Frogskin sunglasses, Oakley Split Thump MP3 Sunglasses, a Puma golf bag and a Just IV Me Official Sundance Shirt. Retail Value $640.
If winner is an eligible minor, prize will be awarded in the name of his/her parent or legal guardian, who must sign and return all required documents.
Sponsor may, at its sole discretion and for any reason, substitute a prize of equal or greater value, including a cash award.
Winner is solely responsible for all applicable Federal, State and local taxes on the prize. Prize is not transferable. To receive prize, winner (and/or his/her parent or legal guardian if the winner is below the age of 18) must execute an affidavit of eligibility, release of liability and prize acceptance form within 10 days of notification, if required by Sponsor. Noncompliance or return of any form as undeliverable will result in disqualification and selection of an alternate winner.
4. DRAWING METHODOLOGY:
Winner will be selected by a random drawing held within 5 days of the conclusion of the entry period set forth herein from among all eligible entries received. The drawing will be conducted by or under the supervision of Sponsor, whose decisions will be final in all matters concerning the sweepstakes.
Winner will be notified by e-mail within fourteen (14) days after the drawing. If winner cannot be contacted or does not respond within five (5) days of being contacted, if any prize is returned as undeliverable, or in the event a potential winner does not accept the prize or fails to comply with these rules and requirements, the prize will be forfeited. Upon prize forfeiture, no compensation will be given.
Except where prohibited by law, by entering, winner consents to the use of his/her name, voice, likeness and/or biographical information for advertising and promotional purposes without additional consideration with the understanding that it will be relied upon by the Sponsor in connection with this sweepstakes.
The Sponsor, its parent company, their subsidiaries, owners, stockholders, agents, licensees, agencies, affiliates, officers, directors, employees, assigns, successors, managers and operators of the sweepstakes, advertising and/or promotional agencies or any other sponsors or promoters (the "concerned parties"), assume no liability or responsibilities, financial or otherwise, of any damage, loss, injury or death resulting from or arising out of the acceptance and use of the prize. Sponsor is not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of the prize. Sponsor reserves the right at any time to cancel this sweepstakes, make rules and contest changes and/or substitute other prizes of equal or greater value for any reason.
5. ELIGIBILITY:
The sweepstakes is open to legal residents of the 50 United States and D.C., currently residing therein. Entrants must be thirteen (13) years of age or older at the time of entry. ENTRANT WHO IS UNDER 18 AND/OR A MINOR IS HIS/HER STATE OF RESIDENCE MUST HAVE PERMISSION OF PARENT OR LEGAL GUARDIAN TO ENTER. Employees of Sponsor, Sponsor's parent company, affiliates, subsidiaries, advertising and promotion agencies directly involved in this sweepstakes, and members of the immediate families or households of any of the above, are NOT eligible to participate in this sweepstakes. Void where prohibited or restricted by law and subject to all Federal, State and local laws.
6. DECISION OF THE JUDGE(S):
All decisions of the Sponsor regarding winner selection and this sweepstakes are final. Sponsor reserves the right to cancel this promotion at any time for any reason without notice.
