Disturbing new details about Cory Monteith’s untimely death have emerged.
TMZ obtained the final coroner’s report, which lists the cause of his accidental death as “mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol.”
The Glee star, who was dating co-star Lea Michele at the time, was found in his hotel room in Vancouver, Canada on July 13. When responders arrived to the scene, they found a “spoon with drug residue and a used hypodermic needle.”

Two empty champagne bottles and glasses were also found.
The report notes that the actor “had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs.”

The coroner believes that Cory’s body couldn’t handle the drug intake because he had stopped using since completing a rehab stint in April — making his tolerance for substance abuse lower than usual.
Glee's fifth season kicked off last Thursday, Sept. 26. An episode dedicated to the death of Cory and his character, Finn, will air next week.

“Filming the episode was incredibly emotional for the cast and crew,” a source tells Life & Style. “Everyone cried. He was loved on that set.”
As for Lea, the insider shares, “She’s strong. Ryan Murphy [the creator of the show] has been a huge support to Lea through all of this.”
