It’s been a big week for people fainting on television! First Wendy Williams passed out on her live talk show, and now Cole Medders is keeling over on the episode of Survivor: Heroes vs. Healers vs. Hustlers airing tonight, Nov. 1!

In PEOPLE’s sneak peek of this week’s installment, we find Cole lying down with his arm over his face, his right hand trembling. As fellow contestant Mike Zahalsky complains about a worm falling on him, Cole stands up, then hits the ground with a sickening thump.

As his tribemates rush to his aid, Cole seems disoriented. “What?” he asks, his eyes wide. “What happened? … I thought I was standing up over there, and the next thing I know, I’m, like, dreaming or something. I don’t know.

Tribemate Jessica Johnston offers him her portion of rice over his objections. “You just passed out!” she says. Luckily for Cole, he has two other Healers in his tribe.

“All of us are scared, but Dr. Mike and I were in control,” Jessica tells us at home. “We knew to get fluid in him, we knew to get food.”

Jessica, a nurse practitioner, tells Cole it’s a “food-related issue” since he’s “depleted.” She also has a personal stake in his wellbeing, as we discover.

“I felt bad for him. I literally just wanted to hold him,” she says. “And in that moment, I realized that I really do care for him a lot more than I thought I did. I’m not ready to play this game without Cole.”

You would think that Cole, a rock-climbing guide, would know how to sustain his body in the wilderness. Perhaps he’s only pretending to faint as part of his master plan? We’ll see who the real Survivor is soon enough…
