Wednesday’s episode of My 600-lb Life took a heartbreaking turn when patient Kirsten Perez revealed she had been gang-raped as a teenager.

The 612-pound Washington native had initially refused to go to therapy after undergoing gastric bypass surgery, but her doctor, Dr. Nowzaradan, threatened to stop her treatment if she didn’t keep her appointments.

“Let me be clear — you go to a therapist or there is nothing more I can do for you,” he told the 38-year-old. “Outside of a medical emergency, we are done. Do you understand me?” In a separate interview, Nowzaradan admitted, “This is not a good situation at all. I’m really disappointed right now in Kirsten… She needs a wake-up call.”

Perez eventually went to see psychiatrist Lola Clay, and shared her brutal gang-rape experience during the session. “You have written a narrative in your mind where you’re just as culpable as the perpetrators,” Clay explained. “Let go of [the] responsibility… It’s important that you skew your thinking toward the positive.”

She recommended a mantra that Perez should be repeating to herself. “I will not abuse my body with food. Above all else, I will love and respect myself.”

Perez also confessed that she’d been facing resentment from her son, Neiko, who wasn’t happy about moving to Houston, TX, for her weight loss procedures. He would lash out by cursing and eating junk food and sugary drinks in front of her. She ended up sending him back home so that she could focus on her health.

By the end of the episode, Perez had lost 170 lbs. “I really didn’t expect her to do nearly as well as she has done with it,” her father Kurt said.
